We are proud to introduce the implementation of our C.A.R.E. Program. C.A.R.E. adds another layer of service to caring for our patients and families. We will assess and monitor the patient’s evolving needs and help families and caregivers locate equipment, and resources. Patients evaluated for hospice care but not clinically appropriate will be automatically enrolled in C.A.R.E.
